French Bulldogs and Heatstroke:
Flat-faced dogs are roughly twice as likely to get heat-related illness and about three times as likely to die from it. But the more useful finding is this: exercise causes far more heatstroke cases than hot cars do, and the median “feels like” temperature on days dogs actually collapsed was around 62°F, not a heatwave. This guide covers the real numbers from the largest study ever done on canine heatstroke, why a flat face breaks a dog’s cooling system, the warning signs in order, and one piece of emergency advice that most articles get dangerously wrong for flat-faced breeds specifically.

Flat-faced vs. normal-muzzled overall: 2.1× the odds (95% CI 1.68-2.64).
Two honest notes. This is UK data. There’s no equivalent US primary-care dataset, so treat the incidence rates as indicative rather than transferable. And you’ll see “14 times more likely” quoted a lot; that figure belongs to the English Bulldog, not the French Bulldog.
The finding that should change how you walk your dog
The same research team published a second 2020 paper in Animals with a deliberately blunt title: “Dogs Don’t Die Just in Hot Cars.”
Across 1,259 heat-related illness events, here’s what actually triggered them:

Exertional heatstroke affected roughly ten times as many dogs and killed eight times as many as hot cars. The authors’ own conclusion: “Campaigns to raise public awareness about heat-related illness in dogs need to highlight that dogs don’t die just in hot cars.”
The public messaging has been overwhelmingly about parked cars. The actual danger, statistically, is a walk.
And it doesn’t take a hot day
This is the single most counterintuitive number in the research. In a 2022 VetCompass analysis, the median wet bulb globe temperature on days when dogs presented with heat illness was 16.9°C, about 62°F. The interquartile range was 14.8-18.9°C. Some cases occurred as low as 3.3°C.
Sixty-two degrees. That’s a pleasant spring morning. The dogs weren’t overheating because the day was hot. They were overheating because they were exercising, and their cooling systems couldn’t keep up with the heat their own bodies were producing.
The research team’s practical guidance reflects this: risk-assess any exercise above 10°C (50°F), and “if in doubt, don’t take them out.”
Why a flat face breaks the cooling system
Understanding the mechanism makes every other decision easier.
How dogs actually cool themselves:
At comfortable temperatures, over 70% of a dog’s heat loss is radiation and convection, simply shedding heat to cooler surroundings. Panting is the backup.
But as air temperature climbs toward the dog’s skin temperature (30-35°C / 86-95°F), radiation and convection stop working. You cannot radiate heat into air that’s as warm as you are. At that point the dog is almost entirely dependent on evaporation.
And dogs are badly equipped for evaporation. They have no functional sweat glands over most of the body. The ones on the paw pads contribute negligibly to whole-body cooling, despite what you’ll read. Evaporative cooling happens in the respiratory tract, across the nasal turbinates, sinuses and tongue. A dedicated organ, the lateral nasal gland, secretes fluid onto the nasal lining specifically to supply water for evaporation. Air is drawn in through the nose, over that moist high-surface-area tissue, and expelled through the mouth.
What brachycephaly does to that system:
A flat face doesn’t remove the turbinates. It compresses the same volume of soft tissue into a much shorter skull. The result is a crowded, folded, partially obstructed nasal chamber. One 2024 study found caudal aberrant turbinates in 87.1% of affected dogs studied, with obstruction that “changes nasal airflow and reduces air/mucosal contact, thereby reducing cooling capacity and prolonging post-exercise recovery time.”
Then it gets worse. Because the airway is narrow, the dog has to work harder to breathe, and that work generates additional heat. A 2023 study in Frontiers in Veterinary Science described this dual deficit precisely: increased airway resistance forces respiratory muscles to work harder, producing extra metabolic heat, while reduced surface area cuts panting efficiency.
It’s a cooling system that becomes a heat source under load.
The strongest evidence this is causal: in that same study, 13 French Bulldogs with severe airway disease were treadmill-tested before and 6-9 months after corrective surgery. Their temperature rise at the end of exercise fell from 0.60°C pre-surgery to 0.30°C post-surgery. Cut in half. Fix the airway, and the cooling works better.
BOAS: how common is it, really?
Brachycephalic Obstructive Airway Syndrome is the umbrella term. Its components include stenotic (narrowed) nostrils, an elongated soft palate, everted laryngeal saccules, a hypoplastic trachea, laryngeal collapse, and aberrant nasal turbinates.

Between half and two-thirds of these breeds have clinically significant airway disease. Not “some.” Most.
US insurance data supports the scale of it. Nationwide analyzed claims across more than 50,000 short-nosed dogs and found French Bulldogs were over 17× more likely to have a BOAS claim, English Bulldogs nearly 12×, and Pugs almost 10×.
The US context: this is a growing problem
The French Bulldog has been America’s #1 most popular breed for four consecutive years (2022-2025), ending the Labrador Retriever’s 31-year run at the top. The longest reign in AKC history. The Bulldog sits at #10, Boston Terrier at #23, Pug at #38.
Nationwide’s insured French Bulldog population grew 435% in ten years.
So: the most popular dog in America is a breed where roughly 59% have clinically significant airway disease and the odds of heat illness are 6.5× a Labrador’s. The population-level exposure is rising fast.
Warning signs, in the order they appear
The VetCompass clinical grading tool, built from 856 real cases, groups signs by severity, and the fatality rates attached to each grade are the reason early recognition matters so much.
Mild, 2.2% fatality. This is where you save your dog.

- • Lethargy; slowing down; lagging behind
- • Reluctance to continue exercising or playing
- • Seeking shade
- • Whining
- • Drooling
Your dog wanting to stop is the warning sign. It is not stubbornness.
Moderate, 5.5% fatality

Overall case distribution was 39.9% mild, 46.1% moderate, 14.0% severe. Around 97% of mild cases survived. Under half of severe cases did.

But here’s what most articles miss. The VetCompass researchers deliberately excluded body temperature from their clinical grading tool, concluding that “it is the duration of temperature elevation that results in clinical pathology, so a single temperature reading alone is poorly diagnostic.” A US board-certified criticalist puts it more directly: a normal temperature does not rule out heatstroke. A dog that has collapsed after peaking, or been partially cooled, can read normal while being in serious trouble.
Act on signs, not on a thermometer.
Emergency first aid, and the part that’s specific to flat-faced dogs
The guidance here changed, and a lot of published advice hasn’t caught up.
The old advice (still on veterinary sites dating from 2008) was to use lukewarm water only, because cold water supposedly causes vasoconstriction and traps heat.
The current position, from the Royal Veterinary College: “Cool first, transport second.” Dr. Emily Hall, lead author on the VetCompass heat research, states it plainly: “Cool the dog quickly, using whatever water you have available provided the water is cooler than the dog.”
The research team’s own myth-busting piece is direct: “the advice to ‘only use tepid/lukewarm water’ is not supported by evidence.” Cold-water immersion is now the recommended approach in human exertional heatstroke, and the canine evidence points the same way. Rapid cooling within the first two hours improves survival.
🚨 The critical exception for brachycephalic dogs
Cold-water immersion is specifically NOT recommended for flat-faced breeds. This is the piece most “new guidance” articles omit, and it matters enormously if you own a Frenchie or a Pug.

For a flat-faced dog, the instruction is pour water over them and get air moving, not put them in the lake.
The protocol
- 1. Stop immediately. Get into shade.
- 2. Cool first, transport second. Don’t put a hot dog straight in a car. Vehicles cool dogs less effectively.
- 3. Pour or spray cool water over the whole body, making sure it reaches the skin, with air movement over them.
- 4. Offer small amounts of cool water if they’re conscious and able to drink.
- 5. Call your vet while cooling. Travel once active cooling is underway.
- 6. Stop cooling once temperature drops below 104°F (103°F for older or unwell dogs). Rebound hypothermia is real.
What not to do:
- • Don’t delay cooling to drive to the vet
- • Don’t leave wet towels sitting on the dog, 51% of UK cases were treated this way and it’s the least effective method. Once a towel dries it insulates and blocks airflow. If a towel is all you have, keep re-soaking it and get air moving.
- • Don’t immerse a brachycephalic, unconscious, elderly, or cardiorespiratory-compromised dog
- • Don’t give NSAIDs or steroids
- • Don’t wait for them to “shake it off”
A sobering compliance statistic: only 21.7% of dogs received any cooling before arriving at the vet, and only 24% were cooled by a currently recommended method. Dogs cooled before arrival presented at an average of 38.6°C. Dogs cooled only at the
clinic presented at 40.9°C.
They need a vet even if they seem fine
Damage declares itself late. Most deaths occur in the first 24 hours, and dogs still alive at 72 hours in one analysis all survived to discharge. Kidney injury markers can worsen at 24-48 hours despite fluid therapy. In one study, creatinine above 1.5 mg/dL at 24 hours post-admission carried 75% mortality.
Apparent recovery is not recovery.
The risk factor you can actually change
Skull shape isn’t modifiable. Weight is, and the evidence on it is stark.
In a study of 54 heatstroke cases, obese dogs had 82% mortality. A 2017 JAVMA study of 105 dogs under heat challenge found body condition score was positively associated with body temperature and negatively associated with tidal volume, and, remarkably, that body condition was a greater determinant of body temperature than breed type.
Being overweight mattered more than being flat-faced.
It compounds too: high body condition score is a significant risk factor for BOAS itself. For a flat-faced dog, excess weight worsens the airway obstruction and raises baseline temperature and reduces breathing efficiency.
The single highest-leverage thing you can do for a Frenchie or Pug is keep them lean.
Pavement: the other summer hazard

Skin destruction can occur in 60 seconds at 125°F, and second-degree burns within 35 seconds of contact.
The 7-second test: press the back of your hand flat on the pavement. If you can’t hold it for seven full seconds, it’s too hot for paws.
This is worth flagging for flat-faced breeds specifically, because they’re low to the ground, closer to the radiating surface than a taller dog.
Vet guidance on warm-weather exercise
Honest flag: there’s no peer-reviewed, quantified exercise prescription for brachycephalic dogs in heat. This is authoritative guidance, not study findings.
AVMA: overweight pets and short-nosed breeds have “higher risk of problems with warm-weather exercise.” Walk during cooler hours. Avoid hot asphalt. Consult your vet before starting an exercise program with a short-nosed breed.
Cornell’s Riney Canine Health Center: the hottest part of the day is roughly 3-5pm. Walk early morning or evening. Avoid strenuous activity on hot days, very humid days, and in early summer before dogs have acclimated.
The VetCompass team: risk-assess exercise above 50°F. Keep dogs fit year-round, because winter inactivity increases summer vulnerability. Acclimatization takes about six weeks. Sudden early-season heat spells are the most dangerous period of the year.
On humidity
Evaporation only works if the air can absorb more water vapor. Humid air can’t. A dog whose only effective cooling route is respiratory evaporation loses proportionally more of its total cooling capacity in humid conditions than a human does, because we still have whole-body sweating and a much larger radiating surface relative to our metabolic load.
Honest caveat: there is no validated canine heat index or temperature-humidity threshold. A 2025 review explicitly identifies this as a gap in the evidence. And the “safe walking temperature” charts circulating online have no veterinary or peer-reviewed provenance. Every version traces back to commercial blogs. We’re not going to reproduce one. Use the evidenced guidance instead: risk-assess above 50°F, follow your local human heat advisory, and watch your dog.
Where a stroller fits
Given all of the above, the logic is straightforward: exertion is the multiplier. Three-quarters of known-cause heatstroke cases were exercise-driven, at a median temperature around 62°F. Remove the exertion and you remove the dominant risk factor while keeping the outing.
That’s the case for a hybrid approach. Your dog walks the cool, shaded, early-morning portion and rides the rest. They still get outside, still get scent and novelty, still come with you. They just aren’t generating heat they can’t shed.
Three honest caveats:
An enclosed stroller is warmer than open air. Ventilation isn’t a marketing feature for a flat-faced dog, it’s the whole point. Look for genuine cross-ventilation, not one small mesh window, and never leave a dog in a stationary stroller in the sun.
Interior width matters more than length for these breeds. Flat-faced dogs are typically low and wide. A carrier long enough but too narrow won’t work.
A stroller is not a substitute for exercise. Keeping dogs fit year-round is explicitly part of the heat-risk guidance. Deconditioned dogs are more vulnerable. Use it to extend outings safely, not to replace them.

Are French Bulldogs really 14 times more likely to get heatstroke?
No, that figure belongs to the English Bulldog (13.95×). French Bulldogs are 6.49× more likely than a Labrador. Both are serious. The 14× number gets misattributed constantly.
At what temperature should I stop walking my flat-faced dog?
There’s no validated threshold, and be wary of any chart claiming otherwise. The researchers who produced the largest heatstroke study recommend risk-assessing any exercise above 50°F (10°C), because the median case occurred at around 62°F. Cornell recommends avoiding the 3-5pm window and being extra cautious on humid days and in early summer.
Is a hot car still the main danger?
No, and this is the biggest misconception in the category. Hot cars accounted for 5.2% of known-cause cases. Exercise accounted for 74.2%. Hot cars remain lethal, never leave a dog in one, but the statistically dominant risk is a walk.
